Offices that issue the Polish ID card

Apply in any city, then collect where you applied

Because you may apply at any municipal office in Poland, you can choose a city that is close to home or work. Each office explains how its ID card counter works, whether you book online, take a ticket or walk in, and what to have with you, such as your current card or passport and a suitable photo. You normally collect the finished card at the same office.

1 office in Chorzów
  • Chorzów

Urząd Miasta Chorzów — Referat Ewidencji Ludności i Dowodów Osobistych

    Rynek 1, parter, pokoje 9 i 11, 41-500 Chorzów

The card is free, and you normally collect it in person at the same town hall where you applied.

Bring ID/passport or the accepted alternative and one 35 × 45 mm photo taken within six months. Extra civil-status/citizenship evidence may be requested to resolve inconsistent records. An electronic start does not remove attendance: applicants aged 12 and over must attend for signature capture within 30 days or the application may be left unconsidered. Confirm biometric and appointment arrangements. Special proxy collection is an exception for qualifying applications taken outside the office, not a general alternative. For a child, also prepare the applying adult’s ID, the child’s existing document if held and evidence of guardianship/curatorship where relevant. Do not apply the adult personal-collection rule to a young child: a parent or guardian collects under the age-specific rules.
