A certificate of where you are registered, for the offices that ask for it
A zaświadczenie o zameldowaniu is an official certificate stating the address where you are registered, and it can include other details held in the national population register, known as the PESEL register. If you have a PESEL number (Poland’s personal ID number) and a Profil Zaufany, the free Polish government login, you can download your own certificate online. You can also ask the city or municipal office, in person, by post or through an authorised representative.
How it works
Order an official certificate or extract showing where you are registered.
Employers, banks, schools and authorities often ask for official proof of your registered address. Depending on the country, this is a registration certificate, an extract from the population register or a household certificate. Check exactly which document and language the recipient needs, as the versions contain different details. Many places let you order it online with your digital ID; otherwise you can get it at the town hall, sometimes for a small fee.

City and municipal offices issue the certificate, and in Warsaw the district offices do. Each office shows how it takes requests, from booked visits to post or electronic applications, and what it needs to know.
