Offices that issue the Polish ID card

Apply in any city, then collect where you applied

Because you may apply at any municipal office in Poland, you can choose a city that is close to home or work. Each office explains how its ID card counter works, whether you book online, take a ticket or walk in, and what to have with you, such as your current card or passport and a suitable photo. You normally collect the finished card at the same office.

77 offices
  • Białystok

Referat Dowodów Osobistych i Spraw Wojskowych

ul. Branickiego 3/5, parter — sala obsługi interesantów nr 1, 15-085 Białystok

Book one visit to apply and a separate one to collect the card, or start the application online.

Bring your existing ID or valid passport; the office can use another photo document to establish identity and citizenship. The mObywatel document cannot establish identity for this application. Supply one compliant 35×45 mm colour photograph taken within six months. For online submission use the required digital photograph and complete the physical signature and fingerprint step within 30 days. Do not assume that starting online completes an adult application.

  • Bielsko-Biała

    Wydział Spraw Obywatelskich i Przedsiębiorczości

    pl. Ratuszowy 6, 43-300 Bielsko-Biała

Book a visit to apply for your first or replacement card, even if you are registered in another town.

Bring a compliant colour photograph and a valid identity card or passport. If neither is available, ask about alternative identity verification; mObywatel alone is not accepted for this check. Staff prepare the application at the counter, so no paper form is needed. Bring your previous card when collecting unless its loss was reported.

  • Bydgoszcz

Referat Dowodów Osobistych — filia Fordon

    ul. Gawędy 5, Bydgoszcz

You can walk in to apply and later collect your finished card at the same branch.

Bring a 35 × 45 mm colour photograph taken within six months and your previous ID, even if expired, or the alternative identification the city accepts, such as a valid passport. mDowód cannot establish identity for this application. Bring originals of any supporting documents attached online. For serious illness or disability preventing attendance, ask about a visit at your location on +48 52 585 82 47; ordinary proxy collection is not available in every situation.

  • Bydgoszcz

Referat Dowodów Osobistych — Jezuicka, przyjmowanie wniosków

    ul. Jezuicka 6, parter, Bydgoszcz

Walk in to apply, but collect the finished card at a separate counter in another building.

Bring a 35 × 45 mm colour photograph taken within six months and your previous ID, even if expired, or the alternative identification the city accepts, such as a valid passport. mDowód cannot establish identity for this application. • Bytom

    ul. Parkowa 2, 41-902 Bytom

Get a free first or replacement card by taking a queue ticket or booking an appointment online.

Bring a compliant photograph and valid ID or passport. Take the existing identity card for collection. If you have no valid document, ask the office how to confirm identity; national guidance allows another photo document for the biometric visit. The office may require evidence of Polish citizenship. A change of registered address alone does not require a new card.

  • Chorzów

Urząd Miasta Chorzów — Referat Ewidencji Ludności i Dowodów Osobistych

    Rynek 1, parter, pokoje 9 i 11, 41-500 Chorzów

The card is free, and you normally collect it in person at the same town hall where you applied.

Bring ID/passport or the accepted alternative and one 35 × 45 mm photo taken within six months. Extra civil-status/citizenship evidence may be requested to resolve inconsistent records. An electronic start does not remove attendance: applicants aged 12 and over must attend for signature capture within 30 days or the application may be left unconsidered. Confirm biometric and appointment arrangements. Special proxy collection is an exception for qualifying applications taken outside the office, not a general alternative. For a child, also prepare the applying adult’s ID, the child’s existing document if held and evidence of guardianship/curatorship where relevant. Do not apply the adult personal-collection rule to a young child: a parent or guardian collects under the age-specific rules.

  • Częstochowa

Referat Ewidencji Ludności i Dowodów Osobistych

ul. Jerzego Waszyngtona 5, 42-217 Częstochowa

Bring your current ID or valid passport, a compliant 35 × 45 mm colour photo taken within six months and any evidence needed for your application reason. If neither identity document is available, check the alternative photo-document rules. Keep the application receipt. The 30-day biometric-completion deadline is separate from the subsequent production period.

Collect your card personally at the accepting municipality unless a statutory exceptional collection arrangement has been agreed.
