Offices for Dutch birth, marriage and death certificates

Pick the city where the event was registered

A certificate can only be issued by the municipality that registered the birth, marriage, partnership or death, and moving to another city later does not change that. Requests may go in online with DigiD, by post or at the counter, depending on the office, and the document reaches you in different ways. A valid ID is required.

Apply with DigiD, book a visit or walk in without an appointment, and apply in writing if you are abroad.

Bring ID; representatives also need signed authorisation and the applicant’s ID copy. Postal applications use the official form plus ID copy, sent to Gemeente Haarlem, Afdeling Klantcontactcentrum, Postbus 511, 2003 PB Haarlem. Check the certificate fee and postal or multilingual-form supplements.
