Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

52 offices
  • Białystok

    Urząd Stanu Cywilnego w Białymstoku

    ul. Branickiego 3/5, 15-085 Białystok

Records already in the electronic register can be issued quickly, while older paper records take longer.

Prepare the application, identity document, payment confirmation where payable and proof of legal interest where required. Give enough information to identify the birth, marriage or death record and ask which copy the receiving authority accepts. Seven or ten working days concern records that need entry into the electronic register; they are not a universal waiting period for every request.

  • Bielsko-Biała

    Urząd Stanu Cywilnego w Bielsku-Białej — Archiwum

    pl. Ratuszowy 5, 43-300 Bielsko-Biała

You can use the archive queue or apply by post, and digitised records can be issued quickly.

Prepare ID or passport, a signed request and any required legal-interest evidence or power of attorney. Confirm applicable exemptions and payment arrangements with USC (civil registry office).

  • Bydgoszcz

Urząd Stanu Cywilnego w Bydgoszczy — Grudziądzka

    ul. Grudziądzka 9-15, 85-130 Bydgoszcz

Bring the summons along if a court has set you an urgent deadline for the certificate.

Prepare identification, the application, event details and payment proof or an exemption basis. Add authority or evidence of legal interest when applicable. An electronic XML or PDF must be passed on electronically: printing it does not create an official paper original. A multilingual short extract is paper-only. The separate EU multilingual standard form costs PLN 17 and accompanies a document; confirm the foreign recipient’s requirements.

  • Bytom

    Urząd Stanu Cywilnego w Bytomiu

ul. Olejniczaka 22, Wejście od ul. Adama Didura, Bytom

The birth, marriage or death does not have to have happened in Bytom for you to request the copy.

Bring the application and identity document; include authorisation or legal-interest evidence if relevant. The office accepts card payment at the workstation; ask about purpose-based exemptions. National online requests use e-Doręczenia (Poland’s electronic delivery system). Electronic copies should be passed on electronically, not treated as valid paper copies after printing; multilingual abbreviated copies are supplied on paper. The standard fee is PLN 22 for an abbreviated copy, including a multilingual abbreviated copy, or PLN 33 for a full copy. State the purpose so staff can assess any statutory exemption. The separate multilingual standard form costs PLN 17 and accompanies the underlying certificate; it is not a certificate on its own. Ask the receiving institution which document it needs before paying.

  • Chorzów

    Urząd Stanu Cywilnego w Chorzowie

    ul. gen. H. Dąbrowskiego 24, pokoje 1, 2, 3, 8 — zależnie od sprawy, Chorzów

This certificate is issued only to the person it concerns, and you call first to arrange the required appointment.

Prepare the specific certificate application, ID/passport and payment proof. The application includes your declaration that no relevant civil-status circumstances are missing from the register, made under criminal liability for a false statement. Tell the registrar if a foreign event or unrecorded change affects the declaration. Payment at the Rynek 1 machine does not move the service counter to city hall.

  • Częstochowa

    Urząd Stanu Cywilnego w Częstochowie

    ul. Focha 19/21, 42-217 Częstochowa

You can ask for pickup from a 24-hour document locker by submitting the required declaration.

Prepare identification, the application, fee evidence and proof of legal interest if needed. A short copy costs PLN 22 and a full copy PLN 33, subject to applicable exemptions. Check which format the recipient needs; multilingual short copies can be requested.

  • Dąbrowa Górnicza

Urząd Miejski w Dąbrowie Górniczej — Urząd Stanu Cywilnego

ul. Graniczna 21, 41-300 Dąbrowa Górnicza

Older paper records can take seven to ten working days, so do not count on getting the copy on the spot.

Prepare form USC/01, identity document and payment evidence. Add authority to act or legal-interest evidence where applicable, plus the event’s names, date and place to help identify the record. Standard copies cost PLN 22 abbreviated or multilingual, or PLN 33 full; a full copy from books kept before 1946 is charged separately at PLN 39. State the purpose and ask about statutory exemptions or archival-document rules. Online applications use e-Doręczenia (Poland’s electronic delivery system); keep electronic copies electronic. Multilingual abbreviated copies are paper documents.

  • Elbląg

Urząd Stanu Cywilnego w Elblągu — odpisy aktów

ul. Łączności 2, Budynek D, parter, pokój 1, 82-300 Elbląg

Ordinary email is only for questions, so send a signed application by post or use the national online service.

Use KU-39/DSO/Z1, show identification, specify the record and copy type, and provide fee proof or your exemption purpose. Prepare relationship, representation or legal-interest evidence when required. The authority fee is PLN 17, subject to exemptions. City payment machines are at Łączności 1, not the USC (civil registry office) visitor address. If receiving an electronic copy, preserve its original electronic file; a printout does not retain its electronic signature.

  • Gdańsk

    Zespół Obsługi Mieszkańców Nr 1

    ul. Partyzantów 74, 80-254 Gdańsk

Book a visit for the civil-record service, or apply by post or through the government’s online service.

Bring identification, the application, proof of payment or the purpose supporting an exemption, and any required legal-interest or guardianship evidence. Representatives need original authorisation; its usual fee is PLN 17, with exemptions including close family.

  • Gdynia

Urząd Miasta Gdyni — Urząd Stanu Cywilnego

al. Marszałka Piłsudskiego 52/54, Archiwum: sala 27; transkrypcja według rodzaju aktu: pokoje 17, 18, 20, 81-382 Gdynia

A multilingual standard form can accompany your Polish certificate, though it has no legal value on its own.

Bring identity, record details, proof of legal interest where required, and original power of attorney if represented. State the purpose before paying: certain passport, benefit, education, employment and ZUS (Poland’s social insurance institution) uses are exempt. Ordinary power-of-attorney duty is PLN 17, with specified close-family exemptions. Keep payment proof. Records already in the central register can be issued promptly; the office’s own older books take up to seven working days and books held elsewhere ten.

  • Gliwice

    Urząd Stanu Cywilnego w Gliwicach

ul. Zwycięstwa 21, Urząd Stanu Cywilnego, parter, 44-100 Gliwice

No appointment is needed, so you can walk in to the certificate counter, or apply by post or online.

Prepare the application with the registered person’s details, event date, purpose and requested format; bring a valid ID card or passport and proof of the applicable payment. An authorised representative needs written authority and identification; ask whether a representation-fee exemption applies. A separate EU multilingual standard form is an attachment to the certificate, not a standalone certificate or a full translation of every full-extract entry.

  • Gorzów Wielkopolski

Urząd Stanu Cywilnego w Gorzowie Wielkopolskim

ul. Kazimierza Wielkiego 1, 66-400 Gorzów Wielkopolski

You do not need to live in Gorzów Wielkopolski to request a copy of a Polish civil record.

Bring valid ID, identify the event and people concerned, and provide fee evidence or the reason for an exemption. Representatives need suitable authority and any applicable proxy fee. If you receive a digitally signed certificate, pass on the original electronic file: a printout is not its legal equivalent. Check the recipient’s required certificate format before paying.

  • Katowice

    Urząd Stanu Cywilnego w Katowicach

    Plac Wolności 12a, 40-078 Katowice

If you choose an electronic copy, forward it as the original file, because a printout is not a substitute.

Prepare the names, event date and place, and record number or original registry if known. Add evidence of legal interest where required, any representative’s authorisation and payment proof. Explain the purpose so the office can assess a fee exemption.

  • Kielce

Urząd Stanu Cywilnego w Kielcach – odpisy aktów

    Rynek 1, 25-303 Kielce

This covers existing Polish records and does not register a birth, marriage or death that happened abroad.

Prepare ID or passport, the person’s details and event date/place, required version and purpose, plus authorisation or legal-interest evidence where applicable. Standard fees are PLN 22 for abbreviated or multilingual paper extracts and PLN 33 for complete copies, with statutory exemptions. An electronic original must be forwarded electronically; printing it does not preserve its legal validity.

  • Koszalin

    Urząd Stanu Cywilnego w Koszalinie

    ul. Dąbrówki 1, 75-055 Koszalin

Births, marriages and deaths are handled in separate rooms, so check which one you need before you go.

Prepare ID, the correct birth/marriage/death application, identifying event details and fee or exemption evidence; acting for someone else may need authority and PLN 17 proxy duty unless exempt. Choose paper collection/post for a multilingual extract: it is not delivered electronically. A digitally issued XML/PDF must remain digital; printing removes its official-document value. A multilingual marriage extract cannot be issued after certain administrative surname changes or return to a pre-marriage surname. Confirm the correct alternative with USC (civil registry office).

  • Kraków

    I Referat Rejestracji Stanu Cywilnego

    ul. Lubelska 29, 30-003 Kraków

Book a visit online or take a ticket on arrival, though tickets run out once the daily limit is reached.

Bring identification, the appropriate personal-collection or postal-delivery form, payment evidence if required, and proof of entitlement where applicable.

  • Kraków

    II Referat Rejestracji Stanu Cywilnego

    ul. Wadowicka 8W, 30-415 Kraków

An electronically signed copy has to stay electronic, because printing it out does not keep its official status.

Bring identification, the appropriate personal-collection or postal-delivery form, payment evidence if required, and proof of entitlement where applicable.

  • Kraków

    III Referat Rejestracji Stanu Cywilnego

    os. Zgody 2, 31-949 Kraków

Online requests need an electronic mailbox for official letters and electronic ID, so set those up before you start.

Bring identification, the appropriate personal-collection or postal-delivery form, payment evidence if required, and proof of entitlement where applicable.

  • Łódź

    Urząd Stanu Cywilnego w Łodzi

    al. Piłsudskiego 100, 92-236 Łódź

Booked visitors get priority, so reserve a visit on the city’s appointment website first.

State the person, event, purpose, number and format of extracts. Prepare identification and payment proof or the basis for exemption; add authority if someone acts for you. Multilingual extracts and EU multilingual standard forms are paper options, not available as the electronic extract. The separate EU form accompanies the certificate and has its own fee; confirm which document the foreign recipient needs.

  • Lublin

Urząd Stanu Cywilnego w Lublinie — Spokojna

    ul. Spokojna 2, 20-074 Lublin

A short extract shows the current entry, while a full copy also includes the original entry and later changes.

Prepare identity, the record holder’s details, event date/place, requested format and any required fee or legal-interest evidence. Standard fees are PLN 22 for a short or paper multilingual extract and PLN 33 for a full copy; statutory purpose exemptions and proxy-fee exceptions can apply. Electronic originals must remain electronic; a printout is not an original certificate. A multilingual extract is paper-only. National times distinguish seven calendar days for an electronic-register record from seven or ten working days for legacy paper records; postal delivery takes additional time.
