Offices for Dutch birth, marriage and death certificates

Pick the city where the event was registered

A certificate can only be issued by the municipality that registered the birth, marriage, partnership or death, and moving to another city later does not change that. Requests may go in online with DigiD, by post or at the counter, depending on the office, and the document reaches you in different ways. A valid ID is required.

Use DigiD in the Netherlands or the form without DigiD from abroad, and check first that Eindhoven holds your record.

For the no-DigiD route attach ID and payment evidence; at the counter bring valid ID and payment. Check whether the recipient needs a full copy or international extract. An international registered-partnership extract is unavailable; ask about a sworn translation or an eligible EU multilingual standard form.
