Offices for Dutch birth, marriage and death certificates

Pick the city where the event was registered

A certificate can only be issued by the municipality that registered the birth, marriage, partnership or death, and moving to another city later does not change that. Requests may go in online with DigiD, by post or at the counter, depending on the office, and the document reaches you in different ways. A valid ID is required.

88 offices
  • 's-Hertogenbosch

    Stadskantoor ’s-Hertogenbosch

    Wolvenhoek 1, 5211 HH 's-Hertogenbosch

Apply with DigiD or at a booked visit, and if you are abroad without DigiD, send a signed letter by post.

For a postal application from abroad, include the event date and place, people’s names, purpose, overseas delivery address, date, signature and a copy of valid ID bearing your signature. Ask the recipient whether an international extract is needed.

  • Alkmaar

    Stadskantoor Alkmaar — Mallegatsplein

    Mallegatsplein 10, 1815 AG Alkmaar

Apply with DigiD for an event recorded in Alkmaar, or book a counter visit if you apply for someone else.

Check the event municipality, record type and language needed. For a representative request bring the authorisation form, a copy of the person’s ID and your own ID. Ask the receiving organisation whether a full copy or international extract is required.

  • Almelo

    Stadhuis Almelo

Haven Zuidzijde 30, 7607 EW Almelo

Apply online or at a booked counter visit, and expect the certificate by post within a few days, not on the spot.

Bring valid ID and a bank card to the counter. A representative needs written authorisation no older than three months and the applicant’s ID copy, as well as their own ID. Confirm that Almelo holds the record before paying: the processing fee is not refunded when no certificate can be issued.

  • Almere

    Stadhuis Almere — Burgerzaken

    Stadhuisplein 1, 1315 HR Almere

Apply with DigiD for the birth record of someone born in Almere, or phone for a counter appointment if you are in a hurry.

Specify the certificate, number of copies, full name and date of birth, full copy versus international extract and delivery address. A written overseas request must be dated and signed, with a safe copy of valid ID; payment instructions follow and the document is sent after payment. An authorised representative brings the completed municipal authorisation form, your ID copy and their own ID. The fee is €17.80. Postal delivery time is not guaranteed; allow time for both payment and post. At the counter, payment is by debit card.

  • Alphen aan den Rijn

    Gemeentehuis Alphen aan den Rijn

Stadhuisplein 1, 2405 SH Alphen aan den Rijn

Check first that the record was created in Alphen aan den Rijn, as moving there later does not transfer it.

Bring your ID and, for an authorised request, the signed authorisation and the other person’s ID copy. Verify the record and municipality: an application to the wrong municipality is not refunded. International copies are offered for birth, marriage and death; registered-partnership records are issued in Dutch.

  • Amersfoort

    Stadhuis Amersfoort — Burgerzaken

    Stadhuisplein 1, 3811 LM Amersfoort

Apply with DigiD for your own or your child’s record, or use the municipal authorisation form for someone else’s.

For a written or email request, include your name, address, purpose and ID copy. A payment request comes first. Documents requested online are posted within five working days; this is dispatch, not guaranteed delivery. Check the current fee and select the correct document: incorrect orders are not refunded.

  • Amstelveen

    Raadhuis Amstelveen

    Laan Nieuwer-Amstel 1, 1182 JR Amstelveen

Apply online with DigiD, or phone to book a town hall visit if you need the certificate sooner.

Have the event type, date, place and the named person’s details ready, and confirm that Amstelveen holds the record. The application from abroad without DigiD (the Dutch digital ID for government websites) requires a copy of valid identification and explicitly excludes a driving licence. Check the required document format before ordering so you receive the right certificate.

  • Amsterdam

    Stadsloket Zuidoost

Anton de Komplein 150, 1102 CW Amsterdam

Apply online or walk in without an appointment, and expect the certificate by post after about three weeks.

Bring valid identification and specify the marriage or partnership year, intended use and whether an international copy is needed. Budget for the current fee. A representative brings the completed authorisation, their own valid identification and yours or a copy or scan. For a postal request, include your contact details, signature and identity copy.

  • Apeldoorn

    Huis van de Stad Apeldoorn

    Marktplein 1, 7311 LG Apeldoorn

Apply with DigiD and get the copy by post, or book a counter visit to receive it on the spot.

Bring valid ID and payment, preferably a bank card. A representative needs their own valid ID, your valid ID and the completed signed authorisation form. For use abroad, ask for the international copy and check the receiving authority’s requirements. Choose the civil record required, rather than a BRP (Dutch population register) population-register extract.

  • Arnhem

    Gemeente Arnhem — Loket Zuid

    Kronenburggalerij 3, 6831 ET Arnhem

You can get your own record, or your partner’s or child’s, online, at a booked visit or by letter.

Specify the record type, subject’s full name and birth date, date of the event in Arnhem, purpose and your name/address. Sign postal requests and include an ID copy. State whether you need a copy or international extract. For proof of your current registered address, request a BRP (Dutch population register) extract instead.

  • Arnhem

    Stadhuis Arnhem — publieksbalies

    Koningstraat 38, 6811 DG Arnhem

Check first that the event happened in Arnhem, as the city cannot issue a record it does not hold.

Specify the record type, subject’s full name and birth date, date of the event in Arnhem, purpose and your name/address. Sign postal requests and include an ID copy. State whether you need a copy or international extract. For proof of your current registered address, request a BRP (Dutch population register) extract instead.

  • Assen

    Stadhuis Assen — Burgerzaken

Javastraat 15, Tijdelijk adres tijdens verbouwing stadhuis, 9401 KZ Assen

Apply online with DigiD, or go to the counter if it is urgent, where the document can be issued on the spot.

At the counter bring valid ID and a debit card. A written request must identify the record and include a copy of valid ID. For use abroad, check whether you need an international extract or a full copy with a multilingual standard form. Confirm the receiving authority’s requirements before ordering.

  • Barneveld

    Gemeentehuis Barneveld — Publiekszaken

    Raadhuisplein 2, 3771 ER Barneveld

Apply online with DigiD, or use the separate English-language forms if you live abroad.

A representative must bring your signed authorisation, your ID copy and their own ID. Confirm the record and municipality before paying. A current marital-status or address statement is a BRP (Dutch population register) request rather than a copy of an event record.

  • Bergen op Zoom

    Stadskantoor Bergen op Zoom

Jacob Obrechtlaan 4, 4611 AR Bergen op Zoom

Order a record that was made in Bergen op Zoom, even if you now live elsewhere, online or at a booked visit.

Bring valid ID. An adult representative needs their own ID, your ID copy and written authorisation signed by both of you. Confirm which record and format the receiving organisation needs before applying.

  • Bovenkarspel

    Gemeente Stede Broec — Burgerzaken

De Middend 2, Gemeentehuis, 1611 KW Bovenkarspel

Order online with DigiD, or phone to arrange a counter visit and wait for the document there.

Prepare valid identity evidence and the event date and place. A representative needs signed written authorization, their valid ID and the applicant’s identity document or copy. Ask whether the recipient needs a full copy or international extract. A foreign pension fund may need an attestatie de vita instead of a birth, marriage or death certificate.

  • Breda

    Gemeente Breda — Stadskantoor

Claudius Prinsenlaan 10, 4811 DJ Breda

Apply online with DigiD, or contact the city first to arrange another way if you cannot use it.

Prepare the record type, names, event date/place, purpose and delivery address. Divorce or partnership-ending copies are requested where the marriage or partnership was originally registered. For proof of your current address, use a BRP (Dutch population register) extract. Confirm the document and language needed by the receiving organisation.

  • Capelle aan den IJssel

Gemeentehuis Capelle aan den IJssel — Burgerzaken

Rivierweg 111, 2903 AR Capelle aan den IJssel

Check which municipality holds your record, then apply online with DigiD or drop in at the document service.

Specify the act, event details and intended use; request the international version if needed. A counter representative brings their valid original ID, your ID copy with a clear signature, and the completed municipal authorisation. Do not assume the ordinary personal DigiD (the Dutch digital ID for government websites) form can be used for every represented request.

  • Delft

    Gemeente Delft — Stadskantoor

    Stationsplein 1, 2611 BV Delft

Apply online with DigiD or your BSN, or book an appointment before you go to the counter.

State the event and person concerned and check that Delft holds the record. Include the signed authorisation form when applying on someone else’s behalf. International birth, marriage and death extracts use ten languages; partnership extracts use four. A multilingual EU form is a separate option. For proof of your current registered address, request a BRP (Dutch population register) extract instead.

  • Den Helder

    Stadhuis Den Helder

Willemsoord 66, 1781 AS Den Helder

Pick the type of record you need and apply online with DigiD, or book the civil-record counter instead.

Bring valid ID and the relevant event details, including the date of marriage or partnership when applicable. A representative brings the signed municipal form or a handwritten signed authorisation, their own ID and your ID copy. For a divorce record, ask the municipality which register holds the act before applying.

  • Deventer

Gemeente Deventer — locatie Bathmen (bibliotheek)

    Schoolstraat 25, 7437 AE Deventer

Book a visit at the library counter, but the certificate is posted to you rather than handed over on the spot.

Bring valid identification to the counter and specify the event and purpose. Confirm whether the recipient needs a full certified copy or an extract and which language it accepts. For another person, check authorisation or the municipality’s family exceptions; those exceptions require a counter application. Lawyers and notaries have a separate online route using eHerkenning level 2+ or higher.
