Offices for fingerprints and residence card pickup in Poland

Some branches serve only their own cases, so check before you travel

Residence cards come from the foreigners’ department of a regional government office, and some regions also run branches in other towns. Choose a city to see whether its office works with online booking, queue tickets or a date agreed by phone, and what to bring.

Apply at the registry or by post when your card is lost, damaged or out of date, and the office sets your visit.

Prepare the completed replacement form, a copy of your valid travel document with the original for inspection, two signed passport-standard biometric photos no older than six months, original payment confirmation and evidence of the reason for replacement. For a minor, include the guardian’s passport copy and show the original. Changed names may require a current marriage record or USC (civil registry office) decision; for loss or theft, a police report is also needed. The 50% reductions require the relevant social-assistance evidence or proof that the child is under 16. Collect personally with a valid travel document and the previous card if issued. Tell the office if your address changes during the case and bring every document specified in the summons.
