    Start the confirmation request at pz.gov.pl and bring a valid identity document, such as a passport, identity card or residence card. The official checks your details against the document and PESEL (Polish national identification number) register. Follow the expiry date shown on your request.

    Choose confirmation at a point on pz.gov.pl, submit the request and attend personally within its confirmation deadline; the national patient guide states 14 days. Bring a valid identity card or passport and have your PESEL (Polish national identification number) and request details available. Foreign nationals must also bring a document proving that a PESEL number has been assigned; a passport alone is not sufficient. Confirmation is free. Keep your password and authorisation codes private.
