Offices that confirm a Profil Zaufany in Poland

Any confirmation point will do, not only the one for your address

Different kinds of offices act as confirmation points, so a city can offer several. Pick yours to see which office it is, how to get seen, whether you take a numbered ticket and which identity document to bring, such as a residence card or passport.

  • Lublin

Lubelski Oddział Wojewódzki NFZ — Sala Obsługi Klientów

    ul. Nadstawna 2–4, 20-120 Lublin

The fund’s customer hall can confirm your identity, and the national patient line can tell you whether to book first.

Choose confirmation at a point on pz.gov.pl, submit the request and attend personally within its confirmation deadline; the national patient guide states 14 days. Bring a valid identity card or passport and have your PESEL (Polish national identification number) and request details available. Foreign nationals must also bring a document proving that a PESEL number has been assigned; a passport alone is not sufficient. Confirmation is free. Keep your password and authorisation codes private.

  • Lublin

    ZUS Oddział w Lublinie

ul. Tomasza Zana 38A, 20-601 Lublin

Get the free in-person identity check done, and ask specifically for profile confirmation.

Bring a valid document that clearly establishes your identity, such as an identity card, passport or residence card. Check that your personal details agree with PESEL (Polish national identification number), and retain access to the contact details used for your request. Follow the confirmation deadline shown by the current application process. You must attend personally for this method; submitting the request alone does not activate the profile.
