Offices that confirm a Profil Zaufany in Poland

Any confirmation point will do, not only the one for your address

Different kinds of offices act as confirmation points, so a city can offer several. Pick yours to see which office it is, how to get seen, whether you take a numbered ticket and which identity document to bring, such as a residence card or passport.

  • Gdańsk

    Gdański Urząd Pracy

    ul. Lastadia 41, 80-880 Gdańsk

The visit is only an identity check, so it does not register you as unemployed or award any benefits.

Prepare or check the request through the official trusted-profile service and bring your valid identity card or passport. Personal details must match. Follow the deadline shown in your pending request and ask which additional evidence is needed for a changed name or other mismatch. Keep the confirmation and check that the profile works before using it for an electronic application.

  • Gdańsk

Pomorski Oddział Wojewódzki NFZ — Sala Obsługi Klientów

ul. Podwale Staromiejskie 69, 80-844 Gdańsk

Ask for identity confirmation rather than a preventive health check, and join the queue well before closing.

Choose confirmation at a point on pz.gov.pl, submit the request and attend personally within its confirmation deadline; the national patient guide states 14 days. Bring a valid identity card or passport and have your PESEL (Polish national identification number) and request details available. Foreign nationals must also bring a document proving that a PESEL number has been assigned; a passport alone is not sufficient. Confirmation is free. Keep your password and authorisation codes private.

  • Gdańsk

    Zespół Obsługi Mieszkańców Nr 3

ul. Nowe Ogrody 8/12, 80-803 Gdańsk

Ring the city’s contact centre first to find out which desk handles the profile and whether you need a reservation.

Prepare the confirmation request through pz.gov.pl and bring your valid identity card or passport. Check that your personal details and PESEL (Polish national identification number) match and follow the deadline shown for the pending request. If you need renewal or invalidation rather than initial confirmation, ask whether this counter handles that specific task. Keep the result and check your profile before using it to sign another application.

  • Gdańsk

    Zespół Obsługi Mieszkańców Nr 4

    ul. Wilanowska 2, 80-809 Gdańsk

The desks for address registration are not automatically the ones for the profile, so ask before you go.
