Offices that confirm a Profil Zaufany in Poland

Any confirmation point will do, not only the one for your address

Different kinds of offices act as confirmation points, so a city can offer several. Pick yours to see which office it is, how to get seen, whether you take a numbered ticket and which identity document to bring, such as a residence card or passport.

2 offices in Elbląg
  • Elbląg

Urząd Miejski w Elblągu — punkt potwierdzający Profil Zaufany, Departament Spraw Obywatelskich

ul. Łączności 1, budynek A, parter, pokój 100, 82-300 Elbląg

You have to appear in person, because sending papers does not complete the identity check.

Submit the confirmation request at pz.gov.pl and check the attendance deadline shown for your request. Bring a valid identity document and the applicable PESEL (Polish national identification number) evidence, and make sure names and numbers match the application. Staff print the request for signing at the point. Keep access to the phone/email used by your account. The service has no charge.

  • Elbląg

Warmińsko-Mazurski Oddział Wojewódzki NFZ — Delegatura w Elblągu

ul. Generała Józefa Bema 18, 82-300 Elbląg

Phone the national patient line before you go, because no ticket cut-off time has been confirmed for the hall.

Choose confirmation at a point on pz.gov.pl, submit the request and attend personally within its confirmation deadline; the national patient guide states 14 days. Bring a valid identity card or passport and have your PESEL (Polish national identification number) and request details available. Foreign nationals must also bring a document proving that a PESEL number has been assigned; a passport alone is not sufficient. Confirmation is free. Keep your password and authorisation codes private.
