Bring a valid identity card or passport and have your PESEL (Polish national identification number) and request details available. Foreign nationals must also bring a document proving that a PESEL number has been assigned; a passport alone is not sufficient. Confirmation is free. Keep your password and authorisation codes private.

    Polish applicants may show a valid identity card, passport or mDowód. EU citizens should prepare their national ID or passport and a document issued by a Polish authority showing PESEL (Polish national identification number). Non-EU citizens should prepare a passport or residence card; the regional instructions also require Polish-authority PESEL evidence where the residence card lacks that number. Check your request details before attending and sign the printed copies only in the employee’s presence.
