Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

Take a queue ticket on arrival, and the building has an accessible entrance and adapted toilets on every floor.

Bring identification and the details of the record. Explain its purpose and check whether you need evidence of legal interest or a representative’s authority. Payment evidence is needed unless exempt. Your spouse, siblings, parents and other direct ancestors or descendants can request the record without separate proof of legal interest; more distant relatives and unrelated applicants must demonstrate that interest.
