Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

A short extract shows the current entry, while a full copy also includes the original entry and later changes.

Prepare identity, the record holder’s details, event date/place, requested format and any required fee or legal-interest evidence. Standard fees are PLN 22 for a short or paper multilingual extract and PLN 33 for a full copy; statutory purpose exemptions and proxy-fee exceptions can apply. Electronic originals must remain electronic; a printout is not an original certificate. A multilingual extract is paper-only. National times distinguish seven calendar days for an electronic-register record from seven or ten working days for legacy paper records; postal delivery takes additional time.
