Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

No appointment is needed, so you can walk in to the certificate counter, or apply by post or online.

Prepare the application with the registered person’s details, event date, purpose and requested format; bring a valid ID card or passport and proof of the applicable payment. An authorised representative needs written authority and identification; ask whether a representation-fee exemption applies. A separate EU multilingual standard form is an attachment to the certificate, not a standalone certificate or a full translation of every full-extract entry.
