Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

A multilingual standard form can accompany your Polish certificate, though it has no legal value on its own.

Bring identity, record details, proof of legal interest where required, and original power of attorney if represented. State the purpose before paying: certain passport, benefit, education, employment and ZUS (Poland’s social insurance institution) uses are exempt. Ordinary power-of-attorney duty is PLN 17, with specified close-family exemptions. Keep payment proof. Records already in the central register can be issued promptly; the office’s own older books take up to seven working days and books held elsewhere ten.
