Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

Older paper records can take seven to ten working days, so do not count on getting the copy on the spot.

Prepare form USC/01, identity document and payment evidence. Add authority to act or legal-interest evidence where applicable, plus the event’s names, date and place to help identify the record. Standard copies cost PLN 22 abbreviated or multilingual, or PLN 33 full; a full copy from books kept before 1946 is charged separately at PLN 39. State the purpose and ask about statutory exemptions or archival-document rules. Online applications use e-Doręczenia (Poland’s electronic delivery system); keep electronic copies electronic. Multilingual abbreviated copies are paper documents.
