Registry offices that issue civil status certificates

Cities differ on requests, older paper entries and who may collect

Every city has its own Urząd Stanu Cywilnego, and the way it works varies: some issue copies of electronic entries quickly, others need more time for older paper ones, and some take requests only by post, online or after a phone call. Open an office to check its options, what to bring and who may collect the certificate.

    Urząd Stanu Cywilnego w Bytomiu

ul. Olejniczaka 22, Wejście od ul. Adama Didura, Bytom

The birth, marriage or death does not have to have happened in Bytom for you to request the copy.

Bring the application and identity document; include authorisation or legal-interest evidence if relevant. The office accepts card payment at the workstation; ask about purpose-based exemptions. National online requests use e-Doręczenia (Poland’s electronic delivery system). Electronic copies should be passed on electronically, not treated as valid paper copies after printing; multilingual abbreviated copies are supplied on paper. The standard fee is PLN 22 for an abbreviated copy, including a multilingual abbreviated copy, or PLN 33 for a full copy. State the purpose so staff can assess any statutory exemption. The separate multilingual standard form costs PLN 17 and accompanies the underlying certificate; it is not a certificate on its own. Ask the receiving institution which document it needs before paying.
