The licence is issued by the transport department for your address

The licence is replaced by the transport department of the city or county office for your address. Some offices hand out tickets at a dedicated counter, others want a reservation online or by phone, and some start you at an information desk. Medical certificates for time-limited licences are another point where offices go their own way.

    Prepare driving-licence form F-SO0611, a compliant colour 35×45 mm photograph taken within six months and proof of payment. If replacing a damaged document or changing details, include a copy of the licence you hold. Bring identity evidence for inspection and a medical opinion if your medical examinations are no longer current. Do not select the separate driver-qualification-card form F-SO0612 for an ordinary driving-licence duplicate. A representative needs authority; the PLN 17 power-of-attorney duty has exemptions including a spouse, ascendants, descendants and siblings.
