Where to renew or replace a driving licence in Poland

Prepare form SK-1.1, identity document for inspection, a compliant colour 35×45 mm photograph taken within six months, proof of payment and the previous driving document where relevant. Medical and psychological opinions are required only where applicable to your case. Check those requirements before arranging tests. For collection, bring the old driving licence and identification personally; ask the department what to do if the old document is unavailable. A foreign-licence conversion needs its own eligibility and document assessment. Ask the information point for the current application and required residence declaration. Older printed forms may still be accepted with a supplemental declaration during a transition period; ask the office which form applies. Meet the residence conditions and submit the current declaration no later than collection or the planned postal dispatch, where that route is allowed. Some theory-test cases require this by the exam day; confirm which deadline applies with the department.
