Registry offices that transcribe foreign certificates

Each office has its own team, phone line and booking routine

Transcription is handled by the registry office, but not every office arranges it the same way. Some ask you to phone the team for your type of certificate, others use an online calendar or accept documents by post, and an appointment often covers one matter at a time. Prepare the original certificate, its apostille or legalisation and the sworn translation, and ask which copies the office also wants.

  • Białystok

    Urząd Stanu Cywilnego w Białymstoku

    ul. Branickiego 3/5, 15-085 Białystok

Phone the registry team before visiting, and ask what you need before paying for translations.

Prepare the correct birth, marriage or death transcription application, identity document, original foreign certificate and payment evidence. The local procedure requires an official Polish translation by an authorised sworn translator or Polish consul, with an exception for qualifying multilingual forms. Ask which exception or authentication rule applies to your document; do not assume every foreign certificate needs the same treatment. Additional earlier civil records may be needed to fill missing details.

  • Bielsko-Biała

    Urząd Stanu Cywilnego w Bielsku-Białej — Archiwum

    pl. Ratuszowy 5, 43-300 Bielsko-Biała

Apply in person, by post, through a Polish consulate, or via an authorised representative.

Prepare ID, the application, a full foreign certificate or a copy certified by its issuing authority, and the required Polish sworn or consular translation. If a marriage record omits the married surname, a declaration before USC (civil registry office) or a consul is needed. Confirm any apostille or legalisation requirement.

  • Bydgoszcz

Urząd Stanu Cywilnego w Bydgoszczy — Grudziądzka

    ul. Grudziądzka 9-15, 85-130 Bydgoszcz

Phone the team for your type of record to arrange the visit, as each appointment covers one matter.

Prepare the relevant application, official foreign original, required official Polish translation, identity document and payment proof. Submitted originals and translations are retained. Birth and marriage instructions reject laminated documents and may require additional original birth records. Confirm translation and authentication rules for the issuing country and document, including any applicable exemption, instead of assuming every foreign record needs the same apostille.

  • Bytom

    Urząd Stanu Cywilnego w Bytomiu

ul. Olejniczaka 22, Wejście od ul. Adama Didura, Bytom

Check with the registry first, because a photocopy alone may not be accepted.

Prepare the appropriate SC-1.9 birth, SC-1.10 marriage or SC-1.11 death application, identity evidence, the foreign record and the required Polish translation by a sworn translator or consul. Alternative international extracts and specially certified copies are available; ask before ordering translations or submitting a copy. Submitted original extracts and translations are retained, so obtain any additional originals you need elsewhere before filing.

  • Chorzów

    Urząd Stanu Cywilnego w Chorzowie

    ul. gen. H. Dąbrowskiego 24, pokoje 1, 2, 3, 8 — zależnie od sprawy, Chorzów

Call to arrange an appointment, or ask how to send your documents by post.

Use the event-specific application and original foreign certificate, accepted official Polish translation or applicable EU multilingual form, plus payment proof. For marriage transcription, surname declarations for spouses and children are made before the registrar. An authenticated electronic submission does not automatically replace the original foreign record.

  • Częstochowa

    Urząd Stanu Cywilnego w Częstochowie

    ul. Focha 19/21, 42-217 Częstochowa

Apply in person at the counter for births, marriages or deaths, each with its own phone line.

Bring the original foreign certificate, identification, application and payment evidence. Provide an accepted official Polish translation or qualifying EU multilingual translation form. Bring authority or interest evidence where required; check representation fees and family exemptions.

  • Dąbrowa Górnicza

Urząd Miejski w Dąbrowie Górniczej — Urząd Stanu Cywilnego

ul. Graniczna 21, 41-300 Dąbrowa Górnicza

Agree an appointment by phone or in the city's online calendar before you visit.

Prepare USC/07 for a foreign birth/death record or USC/08 for marriage, the foreign record, identity document and payment evidence. You need an official Polish translation by an authorised sworn translator or consul; confirm any accepted multilingual-document exception and legalisation requirement for the issuing country before commissioning work. Fee: PLN 50. Card-payment desk 18 closes at 15:00 Monday–Wednesday, 17:30 Thursday and 12:30 Friday; you can also pay by transfer or at the lobby payment machine. Ask which originals remain with the office.

  • Elbląg

Urząd Stanu Cywilnego w Elblągu — odpisy aktów

ul. Łączności 2, Budynek D, parter, pokój 1, 82-300 Elbląg

Ask the specialist desk which procedure fits your record before you pay anything.

Prepare the foreign original civil record or equivalent official civil-status document, identity document and the event-specific application: KU-37 for birth, KU-35 for marriage, KU-36 for death. The cards list sworn Polish or consular translation; ask about accepted multilingual documents and any country-specific authentication before arranging work. Transcription with a full copy is PLN 50; separate correction or supplementation is PLN 39 each, and authority to act has its own fee/exemptions. Payment machines are at Łączności 1. Ask which originals the office keeps.

  • Gdańsk

    Zespół Obsługi Mieszkańców Nr 1

    ul. Partyzantów 74, 80-254 Gdańsk

Book your visit online and bring the original documents, since electronic contact cannot replace them.

Prepare ID, application, payment evidence and an undamaged, unlaminated original with official Polish translation. Convention No.16 multilingual extracts are exempt from translation. Confirm authentication requirements for the issuing country. Marriage records lacking post-marriage surnames need additional declarations. Arrange an interpreter if needed to make civil-status declarations.

  • Gdynia

Urząd Miasta Gdyni — Urząd Stanu Cywilnego

al. Marszałka Piłsudskiego 52/54, Archiwum: sala 27; transkrypcja według rodzaju aktu: pokoje 17, 18, 20, 81-382 Gdynia

Book a visit by phone or online, and Polish citizens should ask whether transcription is mandatory for them.

Prepare the application, identity document, original foreign record, appropriate sworn translation and payment proof. Ask which translation exemption applies to your exact multilingual document rather than assuming every multilingual record is exempt. Authentication depends on the country and applicable rules; covered EU public documents are exempt from legalisation. Where a foreign marriage record omits surname declarations for spouses or children, ask about making them with the application. If you already have a Polish record, ask about adapting names to Polish spelling.

  • Gliwice

    Urząd Stanu Cywilnego w Gliwicach

ul. Zwycięstwa 21, Urząd Stanu Cywilnego, parter, 44-100 Gliwice

Walk in without an appointment, and note that your foreign original and its translation are kept.

Prepare the application matching birth, marriage or death, the original foreign record, valid identity document and the required sworn translation. Ask whether your multilingual document is accepted without translation and which apostille/legalisation exemption applies to the issuing country and document. Prepare the applicable payment: the full copy issued after transcription costs PLN 50. Additional correction, supplementation or representation may cost separately; do not assume the transcription fee covers all requested changes.

  • Gorzów Wielkopolski

Urząd Stanu Cywilnego w Gorzowie Wielkopolskim

ul. Kazimierza Wielkiego 1, 66-400 Gorzów Wielkopolski

Arrange your submission by phone, and do not use the ordinary form for copies of Polish records.

Prepare the application, original foreign civil record or an issuing-authority-certified copy, valid ID and proof of the relevant fee. The local card asks for an official translation by a sworn translator or Polish consul; confirm any accepted multilingual or EU-document exception before commissioning one. Family representatives still need authority; the local card says a handwritten authorisation need not be notarised.

  • Katowice

    Urząd Stanu Cywilnego w Katowicach

    Plac Wolności 12a, 40-078 Katowice

Call the birth registration team to get started, and ask if missing details in your record can be completed.

Prepare the original foreign birth certificate, required original translation and payment proof. The application asks for the person’s birth details and the parents’ names, birth details, citizenship and PESEL (Polish national identification number) numbers if assigned. Check whether missing details need completion from the parents’ Polish records. Confirm translation requirements with the birth team before ordering one.

  • Kielce

Urząd Stanu Cywilnego w Kielcach – odpisy aktów

    Rynek 1, 25-303 Kielce

You do not need to live in Kielce, but phone first to confirm which form to use.

Prepare the relevant application, original identity and civil-status evidence, and the original foreign certificate with an official Polish translation by a sworn translator or Polish consul, or an appropriate multilingual certificate. Ask which translation and authentication rules apply to the issuing country. Originals normally remain in the case file; a foreign certificate can be returned on request after a certified copy is made, if you cannot obtain it again. For a child born within a marriage, if the parents married abroad and their marriage is not yet registered in Poland, the card requires registering the parents’ marriage first. A marriage record without surname declarations needs the relevant declarations. Ask about any proxy-fee exemption; do not assume every representative owes the PLN 17 fee.

  • Koszalin

    Urząd Stanu Cywilnego w Koszalinie

    ul. Dąbrówki 1, 75-055 Koszalin

There is no online application, so phone the transcription desk first to arrange how to apply.

Use USC-09-01. Prepare identity evidence and the foreign original extract or a copy certified by its issuing authority, with an accepted official Polish translation unless an accepted multilingual form removes that requirement. Ask about authentication for the issuing country. For marriage, check declarations about spouses’ and children’s surnames. The foreign record is normally retained after transcription; ask in advance about the limited exception when another copy cannot be obtained. Representatives should use the appropriate birth or marriage authority form and confirm the PLN 17 duty or exemption.

  • Kraków

    I Referat Rejestracji Stanu Cywilnego

    ul. Lubelska 29, 30-003 Kraków

Join the civil registry queue in person, as this cannot be done fully online.

Prepare the matching application, foreign document, official Polish translation where required, identification and payment evidence. Check accepted multilingual alternatives. Arrange an interpreter if you cannot handle the visit in Polish.

  • Kraków

    II Referat Rejestracji Stanu Cywilnego

    ul. Wadowicka 8W, 30-415 Kraków

Appointments are not offered, so take a queue ticket on arrival and avoid leaving it late in the day.

Prepare the matching application, foreign document, official Polish translation where required, identification and payment evidence. Check accepted multilingual alternatives. Arrange an interpreter if you cannot handle the visit in Polish.

  • Kraków

    III Referat Rejestracji Stanu Cywilnego

    os. Zgody 2, 31-949 Kraków

Bring the foreign document, and ask beforehand if you need it back, as the original is normally kept.

Prepare the matching application, foreign document, official Polish translation where required, identification and payment evidence. Check accepted multilingual alternatives. Arrange an interpreter if you cannot handle the visit in Polish.

  • Łódź

    Urząd Stanu Cywilnego w Łodzi

    al. Piłsudskiego 100, 92-236 Łódź

Reserve a visit on the city's site to get priority, and bring the original foreign document.

Prepare the original foreign record, official Polish translation where required, application, identity document and payment proof. The foreign extract is retained and not returned. Ask about the authentication or apostille and translation rules for the issuing country and document; do not assume one rule covers every foreign certificate. Missing Polish-register details may need supplementation. For a foreign marriage without later surname choices, ask about the accompanying declarations.

  • Lublin

Urząd Stanu Cywilnego w Lublinie — Spokojna

    ul. Spokojna 2, 20-074 Lublin

Ask the foreign records team what documents your country requires, then collect the result in person or by post.

Prepare the relevant birth, marriage or death application, identity document, official full foreign certificate and payment evidence. A notarised photocopy is not the official certificate required for transcription. Ask which Polish sworn translation or accepted multilingual document is needed. Foreign marriage records without post-marriage surnames may need declarations; missing data or a foreign judgment can require additional assessment.
