Offices that accept a report of moving abroad

Use the office for the address where you are registered now

The departure report goes to the city or municipal office where you live, and a representative can file it for you with written authorisation. Offices differ in how they take it: some deal with it at the counter after you book, others point you to the national online route.

Centrum Usług Publicznych — Wydział Spraw Obywatelskich

ul. Witolda Pileckiego 1, 45-331 Opole

Join the citizens’ services queue at the public services centre or reserve a time, and the entrance is accessible.

For a counter visit, prepare the departure-abroad form and a valid identity card or passport. A representative must show their own identity document and written authority to make the report; the national procedure says the authority need not be notarised. A parent, legal guardian or appropriate actual carer reports for someone who cannot act independently. For online representation, follow the electronic-authorisation requirements. If the child’s PESEL (Polish national identification number) entry contains the reporting parent’s PESEL, the online service can verify that relationship; other representation cases are forwarded to the municipality for recording.
