Offices that accept a report of moving abroad

Use the office for the address where you are registered now

The departure report goes to the city or municipal office where you live, and a representative can file it for you with written authorisation. Offices differ in how they take it: some deal with it at the counter after you book, others point you to the national online route.

Urząd Miasta Kielce – Ewidencja Ludności i Dowody Osobiste

    ul. Szymanowskiego 6, 25-361 Kielce

Reporting is free, and you can phone ahead to check the registration desk is staffed.

Use the departure-abroad form, rather than an ordinary domestic deregistration form. Bring your identity card or passport; foreign nationals can use a valid travel document or another valid document establishing identity and nationality. A representative needs written authority and their own identity document. Prepare the departure details and the evidence of authority if acting for someone else. Contact-register enrolment offered with the application is voluntary and free.
