Offices that accept a report of moving abroad

Use the office for the address where you are registered now

The departure report goes to the city or municipal office where you live, and a representative can file it for you with written authorisation. Offices differ in how they take it: some deal with it at the counter after you book, others point you to the national online route.

Biuro Obsługi Mieszkańców — Urząd Miasta Katowice

    Rynek 1, 40-003 Katowice

Report your departure by the day you leave, either at a booked counter visit or through the national online service.

Prepare the departure form and ID card or passport. A representative needs written authority and their own ID; parents or guardians act for children and people without full legal capacity. Distinguish permanent departure from a temporary stay abroad when completing the form.
