The departure report goes to the city or municipal office where you live, and a representative can file it for you with written authorisation. Offices differ in how they take it: some deal with it at the counter after you book, others point you to the national online route.

Meant for Polish citizens leaving for over six months, so it does not cover immigration or tax steps for foreigners.
Bring the correct departure or return form and ID/passport; an authorised representative needs original or officially certified authority. Do not use the return form to report departure. Keep confirmation and check tax, insurance and residence-permit consequences separately.
Częstochowa

The national online procedure now works only through an electronic mailbox for official letters, so set one up first.
Bring the departure or return form and your identity card or passport. A representative needs written authorisation and identification. A legal representative or appropriate carer acts for someone without full legal capacity. Online submission requires one of the supported government sign-in methods.
Dąbrowa Górnicza

Cancelling your address registration does not by itself settle tax residence or immigration obligations.
Bring ID or passport and the completed departure notification with the correct intended permanence and departure date. A representative brings their own identity document and signed authority. The free notification is distinct from any separately requested certificate or chargeable representation document.

This is for leaving Poland, not for changing your address within the country.
Complete KU-23/DSO/Z1 and bring ID or passport. State whether the departure is permanent or temporary and provide the relevant departure information. After returning from a notified temporary absence exceeding six months, use the separate return form KU-23/DSO/Z2 rather than leaving the register with the old departure record.

Confirm family reporting and any separate certificate request with the department.
