ZUS branches that help with social insurance registration

Employees, self-employed people and families can all ask in person

ZUS, the Social Insurance Institution, has large branches and smaller local offices in towns across Poland. Pick a city to see which one serves you, how to arrange a visit and what to bring to ask who registers you and which insurance applies, for yourself or your family.

Ask at the public services centre counter which procedure fits your situation before filing anything yourself.

Bring identification and relevant registration correspondence. Prepare your PESEL (Polish national identification number) or identity-document details and the payer’s details. Check whether ZUS (Poland’s social insurance institution) ZUA, ZZA or the family-member procedure applies. Family-member health-insurance notifications use ZUS ZCNA. If your employer is your contribution payer, notify them within seven days of the circumstances requiring the family member’s registration; the payer then has seven days after receiving that information to submit it.
