Book a visit to find out whether you need health cover only or social insurance as well.

Prepare ID, PESEL (Polish national identification number) or identity-document details, payer information and relevant employment or registration correspondence. ZUS (Poland’s social insurance institution) ZUA covers social and health insurance, or social insurance alone; ZZA is for health insurance only, and ZCNA for eligible family members. The usual mandatory-registration deadline is seven days from the insurance obligation, with separate rules for special cases. Payers settling contributions for at most five people may file on paper. Above five, electronic filing is required unless the director of the competent ZUS branch authorises an exception in a justified case. A representative needs authority. For an eligible family member without their own insurance entitlement, inform your payer within seven days of the relevant change; the payer has seven days from receiving the information to file ZCNA. If you are your own payer, file within seven days of the circumstances arising.
