ZUS branches that help with social insurance registration

Prepare ID, PESEL (Polish national identification number) or identity-document details, payer information and relevant employment or registration correspondence. ZUS (Poland’s social insurance institution) ZUA covers social and health insurance, or social insurance alone; ZZA is for health insurance only, and ZCNA for eligible family members. The usual mandatory-registration deadline is seven days from the insurance obligation, with separate rules for special cases. Payers settling contributions for at most five people may file on paper. Above five, electronic filing is required unless the director of the competent ZUS branch authorises an exception in a justified case. A representative needs authority. For an eligible family member, tell your contribution payer within seven days of the circumstances requiring registration; the payer then has seven days from receiving your information to file ZUS ZCNA. If you are your own payer, register the family member within seven days of those circumstances arising. Check first whether the family member has their own insurance or entitlement under coordination rules.
