Book a visit for help with your own insurance registration or with registering family members.

Bring an identity document, your existing PESEL (Polish national identification number) or identity-document details, information about your contribution payer, and relevant employment or registration correspondence. The usual deadline for mandatory registration is seven days from the insurance obligation arising; special cases have different rules. To register an eligible family member, tell your payer within seven days of the circumstances requiring registration. The payer then has seven days from receiving that information to submit the registration. If you are your own payer, register the family member yourself within seven days of those circumstances arising. First check whether the family member has their own insurance or entitlement under coordination rules.
