Offices that issue PESEL numbers to non-residents in Poland

Some take bookings, some use queue tickets, many ask you to phone first

The request goes to the residents’ register desk of a commune or city office. At the desk you may have to quote the law that requires a PESEL, and whether a representative can go in your place depends on the office.

Online booking may not list this service, so arrange your visit by phone with the PESEL counter directly.

Prepare the personally signed SA-077-01 application, the specific legal provision that requires you to have a PESEL (Polish national identification number), proof of identity and citizenship, and any relevant civil-status records; documents in a foreign language need a Polish translation. Proof of employment, such as a work or mandate contract, is needed only if you apply under Article 3(1)(1) of the taxpayer-identification law, not for every legal ground. You can also apply by post, but check first that this is allowed in your case: national rules on applying in person still apply. General information about e-Doręczenia (Poland’s electronic delivery system) or ePUAP does not mean you can submit this paper PESEL application electronically.
