    Prepare identity and address details and a copy of a valid passport or travel document. Employment cases require employer details and the relevant applicant/employer signatures. The form also asks for a Directorate of Labour exemption confirmation, with an exception for the specified spouses; confirm applicability to your case. For taxable-capital-income applicants, the form waives the applicant signature but still requires the identity-document copy.
